QMHP Application Information

To expedite the processing of ONLINE applications for licensure, applicants can now upload many of the required supporting documents required to apply for certification or registration by using the new documents upload feature in the online application portal. Please be sure to download the full instructions located in the online application portal when you apply for more information about this feature. Documents you wish to upload must be uploaded prior to submitting your online application fee. It is preferred that supplemental documents be provided to the Board in PDF file format.

What is the difference between each online application?
- The Qualified mental health professional – trainee (QMHP – Trainee) Initial Application: this application is for those individuals who hold a minimum of a conferred bachelor’s degree with 15 semester hours of human service coursework and have not completed the required supervised experience hours toward QMHP-A or QMHP-C registration within the last 5 years. "Qualified mental health professional-trainee" means a person who is receiving supervised training to qualify as a qualified mental health professional and is registered with the board.
- The Qualified mental health professional-adult (QMHP – A) Initial Application: this application is for those individuals who hold a minimum of a bachelor’s degree with 15 semester hours of coursework in human services and completed the required hours of supervised experience and/or internship/practicum hours. All supervised experience must be supervised by a qualified supervisor (licensed mental health professional or a person under supervision that has been approved by the Board of Counseling, Board of Psychology, or Board of Social Work as a prerequisite for licensure.)
- The Qualified mental health professional-child (QMHP – C) Initial Application: this application is for those who hold a minimum of a bachelor’s degree in human services or special education and have completed the required hours of supervised experience and/or internship/practicum. (115-8 Approved Degrees in Human Services and Related Fields for QMHP Registration) All supervised experience must be supervised by a qualified supervisor (licensed mental health professional or a person under supervision that has been approved by the Board of Counseling, Board of Psychology, or Board of Social Work as a prerequisite for licensure.)
